Completely fill out this booking form and be prepared to send screening information, including a selfie of you with your government ID. Once screened I will immediately delete your photo, this is a one time requirement for our first date.

All bookings also require a small nonrefundable deposit. You may reschedule once before forfeiting deposit should you have to cancel.
